Metabolic comorbidities in Preserved Ratio Impaired Spirometry (PRISm): A systematic review and meta-analysis

Introduction: Preserved Ratio Impaired Spirometry (PRISm), defined as reduced FEV1 (<80% predicted) with a preserved FEV1/FVC ratio (0.70), is linked to increased respiratory symptoms, obesity, cardiovascular comorbidities, and higher mortality compared to matched populations with normal spirometry. Aim: To systematically evaluate the prevalence of metabolic comorbidities in individuals with PRISm versus normal spirometry. Methods: This systematic review and meta-analysis followed PRISMA guidelines and was registered in PROSPERO (CRD42024599355). Searches of Medline, Embase, and Web of Science were conducted up to 28 October 2024. Included studies reported the prevalence of hypertension, diabetes, hypercholesterolaemia, ischaemic heart disease, heart failure, or cardiovascular disease in PRISm compared to populations with normal spirometry. Results: A total of 508 articles were screened, with 16 studies published between 2011 and 2024 meeting inclusion criteria. These studies, conducted in Europe, East Asia, and North America, included PRISm populations ranging from 1,202 to 353,515 participants. Overall, metabolic comorbidities were more prevalent in PRISm compared to controls. Meta-analysis revealed increased odds for diabetes (OR 1.94 [1.49-2.53], 7 studies), hypertension (OR 1.69 [1.23-2.31], 5 studies), ischaemic heart disease (OR 2.09 [1.99-2.20], 5 studies), and hypercholesterolaemia (OR 1.49 [1.04-2.14], 4 studies). PRISm was also associated with higher BMI (MD 1.35kg/m2 [0.77-1.93]). Conclusion: PRISm is associated with a higher prevalence of metabolic comorbidities compared to normal spirometry populations.
