Automated CT segmentation volumes of mediastinal pulmonary artery predicts overall survival in hypersensitivity pneumonitis

Background: The association between CT pulmonary artery volumes and mortality in patients with hypersensitivity pneumonitis (HP) is not known. Methods: Consecutive patients with volumetric CTs from Izmir, Turkey, diagnosed with HP by multidisciplinary team evaluation were studied. Mediastinal pulmonary artery (PA) segmentations were carried out using an in-house deep learning algorithm, and the resultant volumes calculated. Survival analysis was performed, with the Cox proportional hazards model adjusted for age, gender, and forced vital capacity (FVC, % predicted). Results: Of 155 participants included, 99 died during the follow up period. The median age was 65 (IQR 58, 72), and 59% female. The median follow up time was 3 years (IQR 1.4, 5.6) and the median PA volume was 105ml (80ml, 126ml). In survival analysis, FVC was associated with survival (% predicted, HR 0.97 [95%CI 0.96-0.98], p < 0.001), as well as PA volume (cL, HR 1.10 [1.04-1.19], p = 0.002). Interpretation: Fully automated mediastinal pulmonary artery volume measurements independently predict survival in patients with HP.
