Inhaler Device Use And Treatment Compliance On Opposite Sides Of The Aegean Sea

Background-Aim: Patient adherence is an important parameter about effectiveness of COPD treatment. (Test of adherence to inhalers) TAI-12 scoring system, which is used in the evaluation of adherence of patients with obstructive lung diseases, includes ten patient-rated items. The study aims to assess levels of adherence to inhalers, and associations with COPD outcomes in patients living in two sides of Aegean Sea; Turkey and Greece. Material-Method: The cross-sectional study included 248 COPD patients who completed the TAI-12 questionnaire between June-December 2024 from 6 centers in Turkey and Greece (152 Turkish, 148 Greek patients). Participants were also evaluated with St. George Respiratory Questionnaire (SGRQ). Demographic data and features about COPD were recorded. Results: There was a significant difference between Turkish and Greece COPD patients in terms of age (p=0.001), gender (p=0.012), presence of comorbidity (p<0.001), exacerbation history (p<0.001) and adherence according to TAI-12 score (p<0.001). The rate of group E COPD patients and triple therapy usage were statistically higher in Greek COPD group (p<0.001 and p=0.021, respectively). There was a positive correlation of TAI-12 score with duration of COPD (r=0.288, p<0.001), and negative correlation with number of medicine (r=0.288, p<0.001), CAT score (r=-0.146, p=0.012) and SGRQ symptom score (r=-0.213, p=0.002). Inhaler treatment compliance was found to be low in those with cough symptoms (p=0.031). Conclusion: Sociocultural differences, demographics and basic features of the disease have been identified as factors affecting inhaler compliance in COPD, even for two opposite sides of the Aegean.
