Validating the ROME COPD exacerbation classification in a hospitalised cohort

The ROME COPD exacerbation severity score aims to improve care and standardise assessment by grading clinical and biochemical markers of inflammation(Celli, B. AJRCCM2021;11:1251-1258). We describe the feasibility of validating the ROME classification in a large, well phenotyped, real-world cohort of hospitalised patients (Bristol, UK). Methods: We assessed the AvonCAP cohort for case ascertainment, screening criteria and variables to define our COPD exposure group. We stratified a defined group of COPD exacerbations by adapted ROME score using data from 2020-21 only. Results: Between 2020-24 AvonCAP enrolled 42,865 participants (COPD n=12,146) ≥18yo, hospitalised with acute (<24days) community-acquired lower respiratory tract disease. Case ascertainment was >90%. Biochemical and clinical features were documented (excluding PaCO2 and baseline spirometry), including infection surveillance and clinical outcomes (recovery/mortality/complications). A constructed COPD variable (exacerbation of COPD/history of COPD/smoking history/>40yo) showed correlation between ROME severity and CURB-65 score in the first year of data(figure 1). erj;66/suppl_69/PA2550/F1 F1 F1 Conclusions: This is a unique dataset with potential as the largest cohort to date to validate the ROME severity score. It lacks spirometry and PaCO2 which provides evidence of disease and exacerbation severity. If limitations are addressed via data linkage, it could further our understanding of exacerbation endotyping.
