Cardioselective beta-blockers in chronic obstructive pulmonary disease and heart failure with reduced ejection fraction: a systematic review and meta-analysis

Aim: To assess the clinical outcomes of cardio-selective BBs in patients with COPD and HFrEF. Methods: We comprehensively searched databases for articles evaluating clinical outcomes of cardio-selective BBs in patients with COPD coexisting with HFrEF. Results: Four observational studies involving 2,740 patients were included [Figure 1]. The mean follow-up period was 17 months, ranging from 2 to 30 months. Compared to non-cardio-selective BBs, cardio-selective BBs were associated with a reduction in all-cause mortality, however, it was not statistically significant (hazard ratio [HR] 0.92, 95% confidence interval [CI] 0.77-1.11, p=0.40) [Figure 2]. Compared to no BB use, cardio-selective BBs significantly reduced all-cause mortality (HR 0.56, 95% CI 0.36-0.87, p=0.010). Additionally, cardio-selective BBs significantly reduced the risk of HF hospitalization compared to non-cardio-selective BBs (HR 0.72, 95% CI 0.62-0.84, p<0.0001).
