Trust your gut: Are gut microbial species associated with asthma?

Introduction: The gut microbiome is implicated in respiratory disease including asthma, however few large-scale studies assessing its composition in adults with asthma exist. Objectives: To determine if there is a specific gut microbial composition in adults with asthma. Methods: This cross-sectional analysis included 8831 adults, aged 50–64years, from the population-based Swedish CArdioPulmonary bioImage Study. Associations between gut microbiota (exposure) and current asthma (outcome, n=526) were assessed in conditional logistic regression models, adjusted for age, sex, plate, country of birth (basic) smoking status, education, physical activity and fibre intake and BMI (extended). Benjamini-Hochberg corrections (FDR cut-off of 0.10) was used. Results: We found 215 and 134 species in the basic and extended model, respectively. In extended models, each doubling of Streptococcus mutans relative frequency was associated with 6.7% higher odds of asthma (q=1.0*10-6) and each doubling of Lentihominibacter sp905193765 relative frequency was associated with 4.2% lower odds of asthma (q=0.001). Higher microbial diversity was significantly associated with lower odds of asthma; Shannon’s index OR: 0.747 (0.602, 0.925). In sub-analyses, after excluding other respiratory disease (n=8633), chronic diseases (n=4414), use of antibiotics (n=7713), proton-pump inhibitors (n=7741), systemic steroids (n=8425) (all medication use in past 6 months), we found 101, 37, 67, 31 and 66 species associated with asthma, respectively. Conclusion: We found a specific gut microbial composition and lower microbial diversity in adults with asthma. Further studies are needed to determine the benefits of alterations in gut microbiota in asthma.
