Ethnicity and socioeconomic status have no impact on longitudinal spirometry trend for children and young people (CYP) with non-cystic fibrosis bronchiectasis (NCFB)

Introduction: NCFB is an underserved and under-researched disease in children (Chang: ERJ:2021). Longitudinal lung function post-diagnosis is stable (Bastardo:2008). However, the impact of ethnicity and socioeconomic deprivation on lung function is unclear. Objective: Determine the association of ethnicity and deprivation on longitudinal lung function in NCFB. Methods: Demographic, spirometry z-scores, Index of multiple deprivation deciles (IMDD) scores (1-5: more deprived and 6-10: less deprived), ethnicity (using UK census data categories) and 4-year lung function data from CYP with computerized tomography (CT)-confirmed NCFB from regional pediatric bronchiectasis service in UK were analyzed. Linear regression and descriptive statistics were used. Results: Out of 102 NCFB patients, 46 (45.1%) had 4-year consecutive spirometry data. The mean age was 13.68 years (SD ± 2.26) and mean age at diagnosis was 7.13 years (SD ± 2.68). 30 (65.2%) were White and 16 (34.8%) from ethnic groups. 28 (60.9%) were from ‘more deprived’ backgrounds, with 15 (53.6%) of these being from ethnic groups. No changes were noted in FEV1 (-1.55 to -1.40, slope 0.09, CI -0.08 to 0.91, p=0.23) & FVC (-1.22 to -1.4, slope 0.05, CI -0.09 to 0.12, p= 0.34) in the cohort. No differences were noted in FEV1 or FVC z-scores between different ethnic groups or deprivation scores. Conclusion: Lung function remains stable in children and young people with non-CF bronchiectasis, four years after diagnosis irrespective of ethnicity or social deprivation. Further long-term lung function trajectory data may inform management strategies to preserve lung growth.
