Looking Beyond Flu: Superadded Infections in Influenza Patients

Introduction: Seasonal influenza contributes significantly to lower respiratory tract infections (LRTIs), especially in autumn and winter. Secondary infections frequently lead to hospitalization. This study assessed the incidence, trends, and bacterial co-infections in influenza-positive patients in a tertiary hospital. Aims and Objectives: To determine the incidence and microbiological profile of bacterial co-pathogens in influenza-positive patients undergoing bronchoalveolar lavage (BAL). Methods: This retrospective study included LRTI patients undergoing BAL (October 2021–November 2024). Microbiological assessment was performed using RT-PCR via BIOFIRE for influenza and bacterial co-pathogens, alongside conventional cultures. Results: Among 1,068 BAL patients, 228 (21.4%) tested influenza-positive. Bacterial co-pathogens were identified in 178 (78.1%) cases, including Haemophilus influenzae (41 cases, 3 culture-positive), Klebsiella pneumoniae (29 cases, 20 culture-positive), Streptococcus pneumoniae (23 cases, 2 culture-positive), and Pseudomonas aeruginosa (16 cases, 9 culture-positive). Peak influenza-associated LRTIs occurred in August. Conclusions: Influenza frequently coexists with bacterial infections, complicating LRTIs. Targeted diagnostics and early intervention are essential.
