Experience in caring patients on long-term home invasive mechanical ventilation in district hospital in Hong Kong

Introduction: The population of receiving long-term mechanical ventilation is growing worldwide. The objective of the study was to review the management of these patients on Long-Term HIMV in district hospital in Kwun Tong over the past two decades. Objectives: 1. To review of mechanisms to ensure that HIMV patients have access to safe, personalized and continuous care from hospital to community. 2. To describe their characteristics. Methodology: 1. There were a retrospective review. 2. Medical records from the care of the ventilator program at the respiratory ward in United Christian Hospital between 2001 and January of 2025. The key components of the program includes: -Assigned respiratory specialty nurse -MDT holistic assessment, planning, implementation and evaluation -Carer education -Home visit before discharge -Hotline support Results: There were a total of 16 adult cases, including 13 males and 3 female admitted to our unit for management of HIMV from hospital to community since 1999. The age ranged from 22 to 85, all of cases required invasive mechanical ventilation via tracheostomy. Underlying causes included neuromuscular diseases predominance (62.4%); chronic lung diseases (18.8%) and airway problems (18.8%). Despite their medical complexities, 15 of cases could be discharged and resided in the community (93.75%). Unplanned readmission ranged from 0.33 to 4.87 times per year. The person-time ranged from 0.59 to 21.6 per year. Conclusion: The HIMV management program could provide the VAIs quality and continuous holistic care from hospital to community. More clinical outcomes examining the effectiveness of the program would be explored for continuous improvement.
