Switching patients with severe eosinophilic asthma from Tezepelumab to Benralizumab

There are several biologics available for patients with severe asthma who are still symptomatic. In most cases, differential therapy is based on biomarkers. Since Tezepelumab doesn't require biomarker measurement in symptomatic patients in Germany, the drug is sometimes prescribed without these signals. We report on patients referred to our center for severe asthma, who were still symptomatic under Tezepelumab treatment. 12 patients (8 female, 6 male, mean age 46.5 years) were referred and followed up due to history of elevated eosinophils. All were on high-dose triple therapies and intermittent oral corticosteroids (OCS) when they started Tezepelumab. After 4-6 injections, they were referred to the asthma centre due to lack of improvement. The historical eosinophil count was 368 ul and FeNO 35 ppb. After switching, 4 (33%) patients had a rapid response to Benralizumab, another 6 (50%) showed improvement after 6 injections, 3 (27%) had a partial response but couldn't be weaned off OCS (Table 1). No side effects were reported. Switching severe eosinophilic asthma patients on tezepelumab therapy to Benralizumab with historically elevated eosinophils above 300 /ul resulted in significant clinical improvement. erj;66/suppl_69/PA1375/F1 F1 F1
