Validity of the FRAIL scale in COPD

Introduction The FRAIL (Frail, Resistance, Ambulation, Illnesses, Loss of weight) scale is a simple 5-item screening tool used to identify frailty in older adults. Limited data exist on its validity in COPD. Aim To test whether FRAIL identifies worsening lung function, breathlessness, health related quality of life (HRQoL), anxiety and depression and physical functioning in COPD. Methods 68 consecutive COPD patients attending community respiratory clinics completed measurements of spirometry, MRC Dyspnoea scale (MRC), COPD Assessment Test (CAT), Hospital Anxiety and Depression (HAD) scale, 4-metre gait speed (4MGS) and Short Physical Performance Battery (SPPB). Participants were stratified according to FRAIL status (Robust:FRAIL=0; Pre-Frail:FRAIL=1-2; Frail:FRAIL=3-5). Welch’s ANOVA or Kruskal Wallis were used to compare across FRAIL status. Results Mean(SD) age 68.1(11.8) years, BMI 27.2(6.5) kg/m2, FEV1 59.4(21.5) %pred, FEV1/FVC ratio 0.55(0.15), median(IQR) MRC 3(1, 4). All completed the FRAIL questionnaire without missing items. There were no significant differences in age, FEV1% pred and BMI between FRAIL status. With worsening FRAIL status, people with COPD reported higher MRC, CAT, HAD-Anxiety and Depression, slower gait speed and worsening SPPB (Figure 1). Conclusion The FRAIL scale may be a useful simple screening tool in COPD to identify patients with increased breathlessness, poorer HRQoL, higher levels of anxiety and depression, and decreased physical functioning. erj;66/suppl_69/PA1985/F1 F1 F1
