Introduction: Pulmonary rehabilitation (PR) is an effective, guideline-recommended intervention in interstitial lung disease (ILD). However, not all participants achieve meaningful benefits. There is a need to optimise exercise training for people with ILD. Aim: To compare the benefits of high intensity interval training (HIIT) to the typical PR method of moderate intensity continuous training (MICT) in ILD. Methods: An assessor-blinded RCT of HIIT vs MICT during PR, with randomisation stratified for exertional desaturation (SpO2 Results: Baseline characteristics of 131 participants, mean age(SD) 70(9), TLCO%pred 56(20), 44% IPF, 64% male, did not differ between groups. COVID-19 disruption, affecting either PR attendance or follow-up, occurred in 40%. Clinically important improvements in cycle endurance time were evident following PR for HIIT and MICT with no between-group difference (Figure). A similar response was seen for 6MWD (MD -2m 95%C1-34 to 31m) and HRQoL with no between-group difference at end PR. Similar proportions of participants perceived their symptoms improved following PR for HIIT (53%) and MICT (53%).
