Healthcare consumption and care pathway of patients who died from COPD between 2017 and 2022 in France.

Chronic obstructive pulmonary disease (COPD) is the fourth leading cause of death worldwide. However, little is known about the characteristics of patients who died from COPD The aim of this study is to describe the characteristics of COPD patients who died from COPD between 2017 and 2022. Patients with COPD between 2017 and 2022 were identified by ICD10 codes: J42 or J43 or J44 in the French National Health Data System. Patients who died from COPD (direct death) (main cause on death certificate) or from respiratory infection associated with COPD (indirect death) between January 1st, 2017 and December 31th, 2022 were included in the analysis. Drug dispensation within 12 months prior to death was used to describe healthcare consumption. Optimal matching & hierachical classification methods were used for the description of care pathway. A total of 500,673 COPD patients died during the study period. Of these, 67,250 died from COPD (direct and indirect). The mean number of COPD exacerbations within 12 months prior to death was 3.55 ± 4.66. Spirometry or plethysmography were performed in 23,115 (34.4%) within 12 months prior to death. The number of patients who died from COPD with at least one dispensation of triple therapy was 23,407 (34.8%). The number of patients with no inhaled therapy dispensation was 12,464 (18.5%). Four categories of care pathway were observed: mostly SABA and/or SAMA use, mostly LABA and/or LAMA use, mostly ICS with another maintenance therapy and no inhaled therapy. In all groups, exacerbations were observed. These data demonstrate that COPD patients who died specifically from COPD are inadequately treated with specific care pathways.
