Joint trajectories of wheezing, asthma and rhinitis through childhood and adulthood: insights from the INMA and ECRHS cohorts

Although respiratory diseases and symptoms frequently coexist, their joint trajectories are not well known. We aimed to identify joint trajectories of wheezing, asthma and rhinitis separately in children and adults. We used child data from the INMA Valencia and Sabadell cohorts at 7-, 9- and 11-year follow-ups, and data from ECRHS adults from 3 follow-ups ten years apart (median age: 34, 42, 54 years). Wheezing, asthma and rhinitis in the last 12 months were defined by a questionnaire. The trajectories were identified separately in INMA and ECRHS with longitudinal k-means clustering. Respiratory symptoms, FEV1, FVC at the different follow-up were used to describe the trajectories. In INMA (n=947), 6 trajectories were identified (Fig. 1). In ECRHS (n=11135), 7 trajectories were identified (Fig. 2). Multimorbid trajectories had more respiratory symptoms and poorer FEV1 and FVC. Despite some variability, evidence at childhood (INMA) and adulthood (ECRHS) consistently showed that these diseases can remit at any stage of life, may have a late-onset, and that 5-10% of the population follow a fixed multimorbid trajectory. erj;66/suppl_69/PA1531/F1 F1 F1 erj;66/suppl_69/PA1531/F2 F2 F2
