Hung-up ankle jerk in hypothyroidism: An underrecognized clinical sign

ABSTRACT Hypothyroidism is a common and important endocrinopathy and refers to the condition of thyroid hormone deficiency. If untreated, it can lead to serious consequences. Because of the large variation in clinical presentation and general absence of symptom specificity, the definition of hypothyroidism is predominantly biochemical. In the presence of characteristic manifestations, the suspicion of hypothyroidism is not difficult, but in the absence of characteristic manifestations, the diagnosis can be challenging. The hung-up reflex or Woltman sign is not a common presentation but a very specific one. The hung-up reflex refers to the delayed relaxation of the elicited deep tendon reflex. It is most commonly seen in the ankle jerk. In this context, we report the case of a 30-year-old male in whom hypothyroidism was suspected in the presence of a hung-up reflex (Woltman sign).
