Detection of extensively drug resistant tuberculosis among drug resistant Mycobacterium tuberculosis clinical isolates in Botswana

The occurrence and spread of Tuberculosis (TB) that is resistant to most available medicines is a danger to controlling TB. Furthermore, we need to assess treatment outcomes of these patients in order to determine if the TB control programmes are working well. This study was done to find out how common TB which is resistant to treatment with effective drugs called flouroquinolones (FLQ) and second line injectable drugs (SLIDs) is in Botswana and to look at treatment outcomes of patients who have drug resistant TB. Fifty-seven samples were tested for resistance to FLQ and SLIDs using a commercially available kit. Treatment outcomes were extracted from patient files. Fifty-seven samples were tested, of these29%, 58%, 7% and 5% were difficult to treat with one, two, three and four drugs respectively. Of the 57 TB patients 32 (56%) were successfully treated, 5 (9%) died, 1 (2%) defaulted, 3 (5%) failed treatment, 12 (21%) were lost to follow up and 4 (7%) did not start treatment. This study shows the importance of routinely testing patients to see if they have TB which is resistant to some of the TB drugs so that patients can be given correct treatments.
