Comparison of CO₂ Laser Combined with 5-ALA-PDT versus LEEP for Treating High-Risk HPV-Associated HSIL in Women of Childbearing Age: A Non-Randomized Controlled Study.

OBJECTIVE This study evaluated the efficacy of CO₂ laser combined with 5-ALA-PDT versus LEEP for treating cervical HSIL with HR-HPV infection. METHODS Ninety patients with HR-HPV associated cervical HSIL were divided into the LEEP Group (n = 60) and CO₂ laser + PDT Group (n = 30). The efficacies were evaluated at 6- and 12-months’follow-up with HPV testing, cytology, and colposcopy examination. RESULTS (1) At 6-months’follow-up, the pathological regression rate was similar between the CO₂ laser + PDT group and the LEEP group (83.3% vs. 96.7%, P>0.05). The HPV clearance rate was higher in the CO₂ laser + PDT group than the LEEP group (85.5% vs. 63.3%, p=0.02). (2) At 12-months’follow-up, the pathological regression and HR-HPV clearance rates were similarly high in the two groups (P>0.05). CONCLUSIONS The efficacy of CO2 laser + PDT is comparable to LEEP for women with cervical HSIL of childbearing age.
