Prospective evaluation of the efficacy of 5-aminolevulinic acid photodynamic therapy, CO₂ laser ablation, and combination therapy for cervical high-grade squamous intraepithelial lesions: A Propensity-Matched Analysis

OBJECTIVE To evaluate the efficacy of PDT, CO₂ laser, and combination therapy for cervical HSIL with HR-HPV infection. METHODS Ninety patients with cervical HSIL were categorized into the PDT group, CO₂ laser group and combination therapy group, and were matched 1:1:1 using a propensity score. The regression rate of the lesions and the HPV clearance rate were compared between the groups at 6- and 12-months’ follow-up. RESULTS (1): At 6-months’ follow-up, the HR-HPV clearance rates and complete remission (CR) rates of cervical HSIL in the combination therapy group were significantly higher than those in the CO₂ laser group or PDT group. (2): At 12-months’follow-up, the HR-HPV clearance rate and the CR rates in the PDT group and combination therapy group were significantly higher than that in the CO₂ laser group. CONCLUSION PDT is an effective method for cervical HSIL treatment and has showed higher HR-HPV clearance rates in long-term period.
