Psychodynamic Insights and Approach to a Case of Dissociative Identity Disorder

Dissociative identity disorder (DID) is characterized by disruptions in identity, presenting as two or more distinct personality states often associated with early trauma. This case study examines a 20-year-old woman with DID, who experienced alternating personality states triggered by familial conflicts and unresolved childhood traumas, including paternal loss, maternal neglect, and ambivalent attachment patterns. A psychodynamic formulation guided the therapeutic approach, focusing on identifying intrapsychic conflicts, unconscious defenses, and emotional triggers. Treatment included cognitive–behavioral therapy, supportive psychotherapy, and pharmacotherapy with escitalopram and clonazepam, leading to reduced dissociative episodes and improved emotional regulation. The study highlights the cultural and familial influences on dissociative symptoms in India and the utility of psychodynamic insights in addressing complex identity fragmentation. This approach emphasizes individualized interventions to stabilize symptoms.
