The Effectiveness of Non-Technical Skills Training in Surgery: A Systematic Review of Randomized Controlled Trials (Preprint)

BACKGROUND Non-technical skills, such as situational awareness, decision-making, teamwork, and communication, are increasingly recognized as factors influencing surgical outcomes. This systematic review aims to examine the current body of evidence and assess the impact of non-technical skills training in surgery. OBJECTIVE To synthesize randomized controlled trial evidence on the effectiveness of non-technical skills training for surgeons, focusing on changes in validated non-technical skills measures and any reported effects on technical performance and related outcomes. METHODS A systematic review was conducted in accordance with PRISMA Guidelines; a systematic search of PubMed, Embase, Cochrane Central Register of Controlled Trials and Web of Science was performed in June 2025. Eligible studies included all English language, Randomized Controlled Trials (RCTs) that focused on non-technical skills training in surgeons. Each included study underwent an evaluation of methodological quality using the Cochrane risk of bias (2.0) tool. The primary outcome was change in non-technical skills following intervention. RESULTS Nine studies comprising 310 surgeons were included in this analysis. Seven out of nine included studies used various validated measures to show an improvement in non-technical skills after undergoing non-technical skills training. The two remaining studies showed no significant change in non-technical skills but they did show a statistically significant improvement in technical skills. CONCLUSIONS Non-technical skills training has been used successfully across a range of surgical specialties with a positive impact on outcome measures. Skills such as situational awareness, decision-making, teamwork, and communication that are essential in minimizing errors, optimizing team performance, and ensuring patient safety. Future studies may address the impact of non-technical skills training on surgeon performance, surgeon well-being, and patient outcomes. CLINICALTRIAL The protocol was prospectively registered on 21/02/2025 in the PROSPERO International prospective register of systematic reviews (Prospero ID CRD42025636687)
