DISP-15. Does it matter where you are treated? Insights from a national review of United Kingdom (UK) brain tumour treatment, care and research services (2020-2024)

Abstract BACKGROUND Despite advances in brain tumour care, survival rates for the most severe tumours types have improved by only 10% since 1971. Studies show significant variation in outcomes both within and across health systems. To further explore equity of care in the context of the United Kingdom (UK) and identify key challenges, we reviewed national brain tumour treatment, care, and research services. METHODS We collected data from 22 neuro-oncology centres (covering 84% of the UK population) in 2020 and 2024, as part of the Tessa Jowell Centre of Excellence, a UK-wide benchmarking and service improvement programme. Analyses were based on 168 data points spanning the patient pathway, drawn from detailed application forms and feedback from over 3,000 patients. RESULTS Key improvements in services from 2020–2024 included: launch of new precision medicine trials, increased peer review of radiotherapy cases (from 74% to 89% of relevant cases), and expanded access to molecular and genomic testing, with total molecular tests submitted increasing 128% (from 2,946 to 6,730), and centres submitting samples for whole genome sequencing rising from 4/22 to 15/22. Emerging challenges included rising delays in imaging, pathology, and oncological treatment. Centres with at least one patient waiting >42 days to begin radiotherapy increased from 7/22 (32%) in 2020 to 20/22 (91%) in 2024. In both years, gaps were noted in access to specialised rehabilitation, neurology, and neuropsychology services, while patient feedback demonstrated varying access to nurse specialists. Finally, although all centres had clinical trials open in both 2020 and 2024, access varied (2–17 open per centre in 2024), with centres reporting numerous barriers, particularly staffing shortages (77%, 17/22). CONCLUSIONS Data highlight both progress and persistent inequities in UK brain tumour services. Findings will support best practice sharing and targeted national efforts to address barriers to equitable care.
