MENG-03 MANAGEMENT OF MENINGIOMAS IN THE NEUROSURGERY DEPARTMENT OF THE MOTHER CHILD UNIVERSITY HOSPITAL IN BAMAKO.

Abstract Background and Aims Meningiomas are typical benign neurosurgical tumors. However, in our context, their evolution, size and location can make their management difficult. Our aim is to share our experience and lead to recommendations. Method We selected 55 files of patients operated for meningiomas in the Neurosurgery Department of the Mother Child University Hospital in Bamako, over a period of 14 years. Incomplete or unusable files were not retained, nor patients who had benefited from other techniques apart from tumor removal. The clinic, the size of the tumor, the location, the quality of excision, as well as the evolution were noted and analyzed. Results The average age of the patients was 47 years. Women accounted for 61.8% of cases. Seizures and CIH syndrome were present in two-thirds of cases (58%). CT imaging was performed in all cases and MRI only in 1/3 of the cases. These lesions were evenly distributed on both sides, and more than half were large (diameter greater than 40 mm), or even giant (up to 100 mm), often located at the anterior level of the skull base (21.8%). Excision was complete in 90% of cases (Simpson 1 (54.5%) and 2). The Fibroblastic type was the most common. Patients were operated on with favorable consequences in 85% of cases at 6 months. Conclusions Meningiomas are often seen at late evolving stages in our countries, with a large size and notable complications that affect the prognosis. Recovery depends on early diagnosis, access to imaging and care, as well as better management and technical conditions.
