Association of Vibration Perception Threshold with Biochemical Markers, Body Composition, and Family History of Diabetes Mellitus

Background: Peripheral neuropathy, especially diabetic peripheral neuropathy (DPN), is a common complication of diabetes.Obesity, particularly visceral fat, is an emerging risk factor, especially in young Indians, where body mass index (BMI) may underestimate risk.Bioelectrical impedance analysis (BIA) provides a better assessment of body composition.Vibration perception threshold (VPT) testing is a noninvasive method for early neuropathy detection.Objectives: To evaluate the relationship between body composition and VPT in healthy young adults and assess the influence of a family history (FH) of diabetes mellitus (DM). Materials and methods:In this cross-sectional study, 200 MBBS students were grouped based on FH of DM (n = 100 each).Anthropometric measurements, glucose levels, HbA1c, and BIA parameters were recorded.VPT was assessed at 12 plantar sites using a digital biothesiometer.Data were analyzed using t-tests and Pearson's correlation.Observations and results: The FH group had significantly higher HbA1c (p = 0.015), postprandial blood glucose (PPBG) (p = 0.001), muscle mass, bone mass, and total body water.VPT was higher in the FH group (p = 0.059).VPT positively correlated with HbA1c (r = 0.21), BMI (r = 0.223), and bone mass (r = 0.202).Conclusion: Elevated VPT in young individuals with FH of DM may indicate early subclinical neuropathy.BIA and VPT could aid in early risk detection and intervention.
