This case report presents the aesthetic restoration of maxillary anterior teeth utilizing a fully digital workflow, including intraoral and facial scanning, mandibular movement trajectory recording, and digital smile design. In this case, intraoral scanning combined with facial scanning was employed to capture and spatially register the patient’s dentition and maxillofacial anatomical features. A digital smile design system, based on facial anatomical features, accurately determined the gingivo-incisal position of the maxillary central incisors, the inclination of the lip–tongue axis, and the curvature of the gingival margins, while proportional analysis of the lateral incisors optimized the aesthetic appearance of the anterior teeth. Furthermore, a mandibular movement trajectory system was utilized to precisely record the patient-specific incisal guidance angle and condylar guidance angle, parameters that were subsequently applied to guide the design of the restorative lingual morphology, ensuring harmonious integration of functional movement and aesthetic form. The treatment was explained to the patients, and written consent for the procedure, photography, and scientific publishing of the procedure performed was obtained before treatment began. This case successfully completed a complex aesthetic restoration using digital-assisted technology, validating the value of integrated multimodal data in precision dental restorations. It should be noted that the incorporation of additional reference markers during the data registration process can significantly improve the accuracy of intraoral and facial scan data integration. This case offers a comprehensive workflow solution—from data acquisition and virtual design to precise clinical implementation—for complex aesthetic restoration.
