Sir, We read with great interest the article by Sence et al., “Comparison of Tuberculosis Cases in Military Personnel Versus Civilians: A Retrospective Study.” The authors provide a valuable contribution by comparing TB cases in military and civilian populations, highlighting a trend toward shorter diagnostic delay in military personnel and the diversity of Mycobacterium tuberculosis complex species identified in the military setting. The primary objective of the study was to assess diagnostic delays between the two groups. Although the difference in diagnostic delay between the two groups was not statistically significant (49 days in the Military group versus 64 days in the Civilian group, P = .42), interpreting this composite outcome requires caution. Diagnostic delay comprises two distinct intervals: (1) from symptom onset to the first medical consultation and (2) from consultation to confirmed diagnosis. The former largely depends on patient behavior but may be prolonged in cases of limited healthcare access, such as in medically underserved areas or among socially vulnerable populations. The second interval, in contrast, depends on the clinician’s diagnostic reasoning and the availability of appropriate investigations.1 Although the diagnostic delay was the main outcome, the study did not differentiate between these two intervals.
