Pseudotumor Cerebri Syndrome Secondary to Internal Jugular Vein Obstruction

A 27-year-old man presented with voice change, globus sensation, and syncopal episodes. MRI revealed a right-sided vagal paraganglioma, and radiotherapy was planned. Before treatment, he developed transient visual obscurations and papilledema. Repeat imaging showed tumor growth with effacement of the internal jugular vein (IJV), impairing cerebral venous drainage. Lumbar puncture and acetazolamide were initiated to manage intracranial pressure while awaiting surgery and radiotherapy. His symptoms improved, and he made a good recovery. This is the first documented case of a vagal paraganglioma causing Pseudotumor Cerebri Syndrome (PTCS) requiring lumbar puncture decompression whilst awaiting definitive intervention. It highlights the need to consider IJV obstruction as a differential for PTCS.
