Abstract Background and Aims Women with autosomal dominant polycystic kidney disease (ADPKD) presenting three or more pregnancies have been considered at risk for progression of the disease. However, the relationship between pregnancy and long-term maternal renal prognoses in ADPKD remains controversial. The aim of the present study was to evaluate the impact of pregnancy on the progression of ADPKD. Method This was a retrospective cross-sectional analysis on data collected from the medical records of women with ADPKD who were followed up as outpatients, for at least three years. Baseline clinical and laboratory data were analysed, and imaging results comprising Mayo imaging classification (MIC) were retrieved. The slope of the estimated glomerular filtration rate (eGFR) was used for evaluating disease progression comparing nulligest women (G0) with those who had had one or more pregnancies (G1). Patients with stage 5 chronic kidney disease were excluded. Results Ninety-four (94) patients were included as G0 (n = 28) or G1 (n = 66). As shown in the Table 1 below, the median follow-up period was not different between groups. G1 women were older, more obese and hypertensive and exhibited lower baseline eGFR. There has been no statistical difference regarding distribution of MIC between groups. A multivariate analysis to assess the factors determining eGFR slope (data not shown in the Table 1), including all aforementioned parameters, showed no independent association. Conclusion These preliminary data suggest that ADPKD women with one or more past pregnancies did not present a worse renal outcome than the nulligest, despite other risk factors such as age, arterial hypertension, obesity and lower baseline eGFR. The similar distribution of rapid progressors in both groups might have contributed to the lack of difference between them. A larger number of patients and a longer follow-up period are still warranted to confirm the present findings.
