Endoscopic retrograde appendicitis therapy versus laparoscopic appendectomy for uncomplicated acute appendicitis with appendicoliths: a multicentre retrospective cohort study

Recent studies support the role of antibiotic therapy in patients with uncomplicated acute appendicitis. However, appendicoliths, present in nearly 35.0% of patients with acute appendicitis, place such patients at high risk of recurrence1–3. Endoscopic retrograde appendicitis therapy (ERAT) is a new option for management of uncomplicated acute appendicitis3. In brief, after intubation of the caecum by colonoscopy, cannulation of the appendix lumen is achieved using a guidewire-catheter technique under X-ray monitoring. Flushing of pus and appendicoliths is attempted using saline irrigation, with basket extraction of the appendicolith and luminal stenting as required. More recently, visual ERAT (V-ERAT) can obviate the need for X-ray monitoring, facilitating visualization of the appendix lumen using a single-use video scope1,4, yet the safety and efficacy of ERAT in patients with appendicoliths remain unclear. This multicentre retrospective cohort study aims to evaluate the safety and efficacy of ERAT versus laparoscopic appendectomy (LA) for uncomplicated acute appendicitis with appendicoliths.
