Dear Editor, Acute pancreatitis is a common surgical emergency in the UK, with an annual incidence of approximately 56/100 000 people. Although most episodes are mild, a quarter of patients present with organ failure and a high risk of mortality. Gallstones and alcohol account for most cases, but many remain idiopathic even after thorough investigation1. The use of glucagon-like peptide-1 receptor agonists (GLP-1RAs) for the treatment of type 2 diabetes and obesity is increasingly common. They include tirzepatide, semaglutide, and liraglutide, among others. Over 1.5 million patients in the UK are prescribed GLP-1RAs, with this number expected to rise in light of recommendations made by the National Institute of Clinical Excellence (NICE) for the treatment of obesity in selected patients2. Concerns have arisen, however, regarding a possible association between GLP-1RAs and acute pancreatitis. Recent observational data point to an increased risk of patients developing acute pancreatitis, whereas similar evidence from RCTs is conflicted3. This uncertainty highlights the urgent need for robust, real-world evidence to better understand the proposed relationship and to clarify its implications for clinical practice.
