Management of pediatric pancreatic tumors: Single-center case series and review of current literature

Primary pancreatic cancers are rare in children lacking predisposing syndromes. The incidence rate is higher in females and adolescents, with clinical presentation often non-specific, and prognosis and histological features different from adult cases. Between 2014 and 2024, we diagnosed and treated nine cases of pediatric primary pancreatic cancer, including six patients diagnosed with solid pseudopapillary tumor. Most cases presented in a body-tail tumor location, and only one was localized at the head of the pancreas. In all patients, the presenting symptoms were nonspecific, other than one case of acute pancreatitis related to trauma. Surgery was successful in treating all cases with localized disease, however multimodal treatment was required to achieve complete response in the only metastatic case. Post-surgical complications occurred in two patients, with complete response after another surgery. The differential diagnosis of pediatric pancreatic lesions is challenging. Radical surgery is the cornerstone of treatment, with an excellent prognosis in lesions with certain pathologies. In our selected small cohort we observed 100 % overall survival at a median follow-up of 4 years.
