Comparison of binary and nonbinary transgender adults’ health and access to care in South Central Appalachia

Objective The number of transgender people is on the rise, as is legislation limiting healthcare access for transgender individuals. Even without these restrictive policies, evidence shows that transgender people face significant challenges accessing care. Additionally, due to lack of access to care and experiences of minority stress, transgender people report worse health outcomes than their cisgender peers. However, differences in health and access to care between binary and nonbinary transgender people are poorly understood.Method We conducted an online survey of 163 transgender adults in the South Central Appalachian region of the United States.Results We found that nonbinary participants reported worse general health and higher levels of non-affirmation in healthcare settings than binary transgender participants. Both binary and nonbinary participants reported gaps between the amount of desired versus received gender-related care (i.e., hormone therapy, counselling), with the gap being more significant for nonbinary participants.Conclusion We argue that there are key gaps in gender-affirming care for transgender people, especially for nonbinary people. Additionally, there are gaps in provision of gender-affirmation in general healthcare settings. An intersectional approach to the study of gender minority wellbeing should include a focus on differences within this population.
