Re-norming Patient-Reported Outcomes Measurement Information System (PROMIS®) pediatric measures: Population-based reference values for children and adolescents

The Patient-Reported Outcomes Measurement Information System (PROMIS®) pediatric measures are widely used in clinical and research settings around the world. To address temporal changes in self-reported health that can occur over time, such as with a global pandemic, periodic updates help maintain relevance. To that end, we sought to establish new reference values for five PROMIS Pediatric item banks version 3 (GenPop v3) - Anxiety, Depressive Symptoms, Anger, Fatigue, and Peer Relationships. Data from 504 children aged 8–17 years using a probability-based online panel were analyzed. Measures were assessed for unidimensionality using confirmatory factor analysis. Differential item functioning was examined across age, gender, race, and household income to minimize item bias across groups. Item banks were recalibrated using a graded response model, and transformed to align with a general population metric. Score distributions from the previous version (v2) and current re-normed version (GenPop v3) were compared using Bland-Altman plots and percentile analyses. The GenPop v3 demonstrated strong psychometric properties, with improved unidimensionality and minimal DIF. Score comparisons revealed that v2 reflected lower symptom severity, particularly at lower symptom levels. Fatigue exhibited the largest discrepancies, while Peer Relationships remained largely stable. The revised item banks ensure alignment with a general pediatric population, improving measurement precision in future use. The PROMIS Pediatric GenPop v3 item banks provide updated, reliable, and standardized assessments of pediatric health outcomes. These refinements enhance the interpretability and accuracy of PROMIS measures in clinical and research settings, ensuring their continued relevance in assessing children’s well-being in a changing health landscape.
