This case report aims to emphasize the importance of recognizing biliary atresia as a potential cause of neonatal cholestasis in newborns with gastroschisis. We describe a neonate diagnosed with the concurrent occurrence of type 1 biliary atresia and gastroschisis. The development of direct hyperbilirubinemia during clinical management prompted further investigation into the causes of neonatal cholestasis, ultimately leading to the diagnosis of type 1 biliary atresia. To our knowledge, such an association has been rarely reported in the literature. This case highlights the need to consider and rule out anatomical causes in the differential diagnosis of neonatal cholestasis, even in the presence of other congenital anomalies such as gastroschisis.
