Based on these findings we used high-dimensional flow cytometry to profile PBMCs of patients with preinvasive disease.We find that this CD39 + CD8 T cell population tracked LUSC development and was enriched in patients with high-grade preinvasive disease.Within the CIS lesions we identified CD8 T cells exhibiting a neoantigen reactive phenotype and markers of early exhaustion. Conclusions:These findings indicate that T cells engage in immune surveillance and acquire features of dysfunction prior to the onset of invasive disease, establishing a foundation for novel immune-interception approaches and advancing a precision immunological framework for early detection and prevention of lung squamous cell carcinoma.
