We present a 63-year-old man with mid-ventricular obstructive HCM and exertional dyspnoea despite maximal beta-blocker therapy. Initiation of Mavacamten, a myosin inhibitor, as an off-label treatment led to significant symptom relief and gradient reduction. This case highlights the potential role of Mavacamten in mid-ventricular obstruction, underscoring the need for further trials to evaluate its broader clinical application in this subset of HCM patients.
