The impact of intermittent fasting on systemic lupus erythematosus disease activity in a cohort of SLE Egyptian patients

Abstract Background Systemic lupus erythematosus (SLE) is a common autoimmune disorder that is primarily managed with immunosuppressive medications. Recent studies have investigated dietary interventions, including intermittent fasting, to help manage chronic inflammation in SLE. Aim The purpose of this study was to investigate the impact of Ramadan fasting on SLE activity in patients with quiescent SLE. Methods We designed a retrospective cohort study involving 100 SLE patients during the holy month of Ramadan under steroid management. All patients underwent thorough history taking and a comprehensive rheumatological examination, with special emphasis on assessing disease activity through the SLEDAI score, laboratory work, and treatment adherence, both before and after fasting was completed. Results No significant difference in clinical activity before and after fasting, except in the non-adherent patients to treatment. Conclusion Our findings suggest that Ramadan fasting can be a safe and beneficial practice, especially for individuals in the milder stages of the disease who strictly adhere to their treatment plans. For Muslims with SLE, Ramadan fasting can promote spiritual fulfillment without detracting from their quality of life.
