Spontaneous vs IVF-conceived Twin Pregnancy: A Hospital-based Cohort Study of 407 Patients

Aims: Multifetal pregnancy is a high-risk pregnancy.The most common perinatal risk is prematurity followed by low birth weight.The aim of this study was to find out the maternal and fetal outcomes in spontaneous and in vitro fertilization (IVF)-conceived twin pregnancies.Objectives: To compare the maternal and perinatal outcomes in spontaneously conceived vs IVF-conceived twin pregnancy.Materials and methods: This is a prospective cohort study done in the Department of Obstetrics and Gynecology from the duration of October 2021 to April 2023.All multifetal pregnancies either IVF conceived or spontaneously conceived were included in the study.Perinatal outcomes in terms of complications, morbidity, and mortality were noted.SPSS version 23 was used for analysis.Results: The total number of multifetal pregnancies in 19 months is 407.So, the incidence is 2.7% (407/15,060).Out of which, 87.96% (358/407) were spontaneously conceived (group A), while 12.03% (49/407) were IVF conceived (group B).The mean age for group A is 27.1 years, while for group B, it is 34.18 years.Group A had 77% preterm labor, while group B had 88%.The requirement of NNU admission was 11.34% for spontaneously conceived multifetal pregnancies while it was 36.36% for IVF pregnancies and this difference is statistically significant.Discussion and conclusion: In vitro fertilization-conceived multifetal pregnancies are definitely more complicated than spontaneously conceived in terms of poor perinatal outcome both perinatal morbidity and mortality.
