Surgical management of extensive jejunal diverticulosis complicated by diverticular perforation and severe anemia in an elderly patient

Jejunal diverticulosis is a very rare clinical entity and is identified in 0.02% to 0.42% of standard radiological studies. We reported the case of perforated diverticulitis in an elderly patient with extensive jejunal diverticulosis. Urgent computed tomography (CT) scan showed a visceral perforation of unclear origin. During laparotomy, we found a diffuse diverticulosis involving jejunum entirely and part of ileum. At 20 cm from Tretz’s ligament we found the perforated diverticulum with enteric spillage and an abscess formation. A segmental jejunal resection involving the perforated area was performed, followed by a side-to-side mechanical anastomosis. The postoperative course was uneventful, oral intake was started in 4th postoperative day and the patient was discharged on the 7th postoperative day. Histologic examination revealed a true diverticulum of jejunal wall with full-thickness wall perforation.
