eP85 Successful Polypectomy of Colonic Polyps Using FTRD: A Case Series

Abstract Aim The aim is to evaluate the effectiveness of the Full-Thickness Resection Device in managing colonic polyps, particularly those with high-grade dysplasia, significant fibrosis, or recurrence, and to assess the complication rates associated with its use. Methods A retrospective analysis was conducted on three patients who underwent FTRD-assisted polypectomy for colonic polyps. The patients were evaluated for underlying conditions, polyp characteristics (size, location, histology), and the success of FTRD in achieving complete resection. Colonoscopy and biopsies were performed prior to the procedure to confirm the histopathological diagnosis. FTRD was used when conventional methods such as standard polypectomy and endoscopic mucosal resection (EMR) were inadequate. Outcomes, including resection success and complications, were recorded. Results Case 1: A 61-year-old woman with a sessile polyp in the distal sigmoid colon exhibiting high-grade dysplasia. After an unsuccessful EMR attempt, FTRD resection was performed with complete excision and no complications. Case 2: A 74-year-old woman with tubular adenomas showing high-grade dysplasia. FTRD confirmed the presence of early-stage well-differentiated adenocarcinoma, prompting further oncological management. Case 3: A 65-year-old man with a tubular adenoma and low-grade dysplasia. FTRD was successful, but he later developed an appendicular abscess, which was managed conservatively without further surgery. Conclusion FTRD is an effective and safe technique for resecting colonic polyps, particularly with high-grade dysplasia or recurrence. It offers a promising alternative when conventional techniques fail, with minimal complications. However, follow-up colonoscopy is essential to monitor for recurrence and confirm complete resection.
