Interstitial High-Dose-Rate Brachytherapy for Radiorecurrent Atypical Meningioma: A Brief Report

Purpose To evaluate the efficacy of interstitial high-dose-rate (HDR) brachytherapy as a novel treatment for a multiply radio-recurrent atypical meningioma. Methods and Materials A 76-year-old female patient with a long history of multiple recurrent atypical meningiomas underwent stereotactic biopsy and HDR brachytherapy. Intraoperative AIRO CT imaging and BrainLAB Varioguide were used to define trajectories. After a stab incision and Stryker twist-drill created a trephine within the skull, stereotactic biopsies were obtained, and two 4 French (4F) brachytherapy catheters (Best Medical) were inserted. HDR brachytherapy was delivered using an Iridium-192 source (Elekta Flexitron), administering 21 Gy in one fraction to the left parietal recurrent atypical meningioma. Results HDR brachytherapy resulted in stable disease control beyond two years. Conclusions HDR brachytherapy offers an effective treatment for multiply recurrent atypical meningiomas. This treatment modality may be considered for other recurrent intracranial tumors.
