Introduction: Acute appendicitis, affecting approximately 7% of individuals, is a common cause of right iliac fossa pain. However, diagnosing appendicitis can be challenging due to similar symptoms in other conditions, leading to high rates of negative appendectomy, particularly in India. Diagnostic methods include clinical scoring systems and imaging techniques, with the Appendicitis Inflammatory Response (AIR) score being a promising tool. The AIR score has shown better specificity and predictive value compared to the Alvarado score, particularly in high-probability cases. In resource-limited settings, scoring systems like AIR and Alvarado are essential for accurate diagnosis and timely surgical decisionmaking. Aim & Objective: Aim: To compare the diagnostic performance of the AIR score and Alvarado score in patients with suspected acute appendicitis in the Bundelkhand region. Objectives: To assess the diagnostic value of the AIR and Alvarado scores. To compare the prognostic value of both scoring systems. Methodology: This hospital-based observational study was conducted at Rajkiya Medical College (Orai), Jalaun, and other medical facilities in Bundelkhand. It involved 100 patients aged 18-50 with suspected acute appendicitis. Both AIR and Alvarado scores were applied, and statistical analysis was performed using Student's t-test and chi-square test, with a significance level of p<0.05. Results: Of the participants, 57% were male, and 43% were female. The majority of patients (62%) were classified with high probability using the Alvarado score, and 55% had intermediate scores using the AIR score. Both scores showed high diagnostic reliability, with statistically significant results (p<0.001 for AIR, p=0.002 for Alvarado). Conclusion: Both the AIR and Alvarado scores were effective in diagnosing acute appendicitis, with higher scores correlating with a higher likelihood of the condition. The study confirms the utility of these scoring systems in clinical practice, particularly in resource-limited settings, aiding in early diagnosis and appropriate management.
