Actors, contributory factors, and preventative strategies for work-related violence in the Australian residential disability sector: A systems analysis

Work-related violence (WRV) in care and support disciplines is a psychosocial risk that can have sizable personal, societal, and economic costs. WRV in residential disability services is a difficult to manage problem due to the complex interplay of multi-factorial contributing factors. Systems thinking models and methods are effective in understanding and responding to complex issues like WRV but have not yet been applied to understand WRV in the residential disability sector. This study applied three systems thinking methods (ActorMap, AcciMap, and PreventiMap) in consultation with 31 stakeholders from the Australian residential disability system and a steering committee to understand the problem. The ActorMap method identified 66 actors who share responsibility for safety in the Australian residential disability system, the AcciMap method identified 90 contributory factors which interacted to produce WRV in the sector, and the PreventiMap method identified 50 interventions (existing or desired) to prevent or reduce WRV in the residential disability system. The results show that contributory factors interact across the system (beyond residents and frontline staff) to produce WRV and suggest that significant intervention is required. Three key themes or leverage points for intervention were identified including staff capabilities, safety culture and leadership, and resident compatibility. The system thinking maps can be used by a decision makers to drive systemic change in the area, using the three key themes to prioritise actions.
