Experience and Reflection on the Treatment of Calcium Channel Antagonist Poisoning: Two Case Reports of Elderly Patients

This article reports two cases of poisoning by calcium channel blockers (CCB) in elderly patients.Case 1 was an 86-year-old female who ingested 48 tablets of amlodipine (240 mg) by mistake, resulting in refractory distributive shock and multiple organ failure.Despite treatment with plasma exchange combined with continuous renal replacement therapy (CRRT), high-dose insulin and calcium, she eventually died due to the severity of her condition.Case 2 was a 77-year-old female who took nifedipine and estazolam in an attempt to commit suicide.She recovered after comprehensive treatment including CRRT, high-dose insulin glucose therapy (HIET), and catharsis.The comparison of the two cases shows that early identification, timely catharsis to remove residual drugs from the gastrointestinal tract, and the rational application of HIET and blood purification techniques are crucial for the treatment of CCB poisoning.The article also discusses the pathophysiological mechanism of CCB poisoning, treatment difficulties, and the particularities of CCB poisoning in elderly patients.jabbrv-ltwa-all.ldfjabbrv-ltwa-en.ldfKey
