Obesity is defined as a “chronic, progressive, relapsing, multi-factorial neurobehavioral disease, wherein an increase in body fat promotes adipose tissue dysfunction and abnormal fat mass physical forces, resulting in adverse metabolic, biomechanical, and psychosocial health consequences” by the Obesity Medicine Association. The condition of obesity presents a significant challenge in primary care due to its complex etiology and numerous associated comorbidities—such as type 2 diabetes, cardiovascular disease, sleep apnea, joint health, and certain cancers. Only recently has obesity been recognized as a disease of metabolic dysregulation rather than a result of poor personal choices. The effective management of obesity requires a comprehensive approach combining intensive lifestyle interventions with additional medical interventions, including medication optimization. This chapter focuses on screening, diagnosis, treatment, or referral of obesity in primary care using people-first language while incorporating lifestyle medicine with additional interventions to provide sustainable, patient-centered care. By highlighting evidence-based strategies and practical applications, this chapter aims to equip primary care providers with the tools necessary to address obesity and improve patients’ outcomes, as well as the referral of complex patients to specialized obesity care and resources for additional provider and patient education.
