81 Prescribing of Appropriate Dual Antiplatelet Therapy Following Emergency Coronary Artery Bypass Grafting Procedure Audit

Abstract Aim Dual antiplatelet therapy is an integral component of post-operative management following an emergency coronary artery bypass grafting procedure. Our aim was to assess whether patients undergoing coronary artery bypass grafting at Essex Cardiothoracic Centre were discharged with appropriate 1-year dual antiplatelet therapy according to EACTS guidelines. Method Discharge records were retrospectively reviewed between May-August 2023 to identify patients who underwent emergency coronary artery bypass grafting. Subsequently, eligible patients discharge summaries were reviewed to establish post-operative antiplatelet therapy status. The intervention implemented involved providing departmental education on dual antiplatelet guidelines to resident doctors and placing posters in the doctor’s office to serve as a point of reference. In addition, a registrar would counter-check antiplatelet medication on the day of patient discharge. A re-audit of patients discharged between September-November 2023 was conducted to assess the effects of the aforementioned intervention. Results Our results demonstrated, prior to intervention, 56 patients underwent emergency coronary artery bypass grafting between May–August 2023. Of which, 82.14% of patients had appropriate post-operative dual antiplatelet therapy on discharge, however 100% of patients had one antiplatelet drug prescribed on discharge. Following intervention, 55 patients underwent emergency coronary artery bypass grafting with 100% of patients being discharged with appropriate post-operative dual antiplatelet therapy. Conclusions Dual antiplatelet therapy following coronary artery bypass grafting is vital to prevent short term post-operative complications and improve long term outcomes. This audit demonstrates how departmental education and introduction of a check prior to discharge can help to improve achieving optimal dual antiplatelet prescribing.
