319 Diagnostic Laparoscopy in Penetrating Abdominal Trauma: How We Do It?

Abstract Background Stab wounds to the abdomen are commonly managed with selective non-operative approaches. In hemodynamically stable patients, diagnostic laparoscopy offers a minimally invasive method to assess intra-abdominal injuries and guide management. Case Presentation A 36-year-old male presented with a stab wound to the left lower quadrant. The patient was hemodynamically stable upon admission. A diagnostic laparoscopy was performed, revealing a Grade I small bowel (SB) injury, which was successfully oversewn. Additionally, bruising was noted at the transverse colon mesentery, prompting further exploration by opening the lesser sac to assess for potential injuries to the pancreas, inferior vena cava (IVC), or stomach. No other injuries were found. This video will demonstrate step by step method of performing diagnostic laparoscopic , following the possible trajectory of Injury and ways to avoid missed injuries. Conclusions This case highlights the utility of diagnostic laparoscopy in stable patients with abdominal stab wounds. The approach facilitated precise identification and repair of injuries while avoiding unnecessary laparotomy. Such minimally invasive methods should be considered in stable patients to minimize morbidity associated with more invasive procedures.
