Background: Prevalence rates of Rheumatoid Arthritis (RA) in Northern Europe range between 650 and 850 cases per 100,000 inhabitants, while rates from studies in Mediterranean areas tend to be slightly lower (between 500 and 750 cases per 100,000 inhabitants) [1]. There are no recent population-based studies in the region evaluating whether the prevalence and incidence of RA have changed. Objectives: Our objective was to determine the prevalence and incidence rates of RA in Catalonia (Spain) during the period from January 1, 2006, and December 31, 2021. Methods: We conducted a population-based cohort study of all existing Catalonian cases who received RA diagnosis from 2006 to 2021 by using ICD-10-CM codes from the Information System for the Development of Research in Primary Care (SIDIAP). A database of primary care electronic health records that includes data from 328 primary care practices covering 5.8 million people, 75% of the Catalan population. Prevalence rate was defined as the number of affected persons in the population at a specified time divided by the number of persons at that time. The numerator of prevalence rate was the number of persons, within 5-year age-sex groups, who met the definition of RA between January 1, 2006, and December 31, 2021, and alive and registered with the SIDIAP on Dec 31, 2021. Incidence rates by age and sex were obtained for the 1-year period between January 1, 2011, and December 31, 2021. Persons diagnosed with RA during the 5-year run-in period from Jan 1, 2006, until December 31, 2010 were not be eligible to become incident cases. Results: We identified 41,152 prevalent cases of RA, with a mean age of 55.8 years (SD 16.2). Of these patients, 29,417 (71.4%) were female, and 26,314 (63.9%) had Spanish Nationality. We found an overall prevalence rate of 393 per 100,000 (Female 564, Men 218). We found a significant increase in prevalence rate during our study period from 257.7 per 100,000 at 2006 to 453 per 100,000 at 2021(p for trend, poisson model <0.0001). Sex-specific and overall prevalence rates by year are depicted in Figure 1. By age groups, prevalent cases were 2,091 (5.0%) in the 18-29 yrs group, 4,367 (10.6%) in the 30-39 group, 6,633 (16.1%) in 40-49 group, 9,095 (22.1%) in 50-59 group, 8,157 (19.8%) in 60-69 group, 7,151 (17.3%) in 70-79 group and 3,658 (8.9%) in group older than 80 years. Overall incident cases during the study period were 13,525. Overall incident rate was 22.2, per 100,000 (Female 31.1 and 13.1 for males). Incidence rates during our study period were stable (22.5 in 2011 and 27.7 in 2021, p for trend, poisson model p=0.189). We found a higher incident rate in 60-69 and 70-79 groups. Sex-specific and overall incidence rates by year are depicted in Figure 2A and by age groups in Figure 2B. Conclusion: This is the first study assessing the prevalence and incidence of RA in Catalonia at the population level. We found an increase trend of the prevalence of RA in our study period. Prevalence rates were 2.6 fold higher in females than males. It is worth noting the highest number of incident cases among the population aged 60-69 and 70-79 years.
