Flexible bronchoscopy (FB) has become an essential tool in critical care, often serving as the procedure of choice for evaluating upper and lower airway pathologies. In conscious patients, FB is considered low-risk. However, in critically ill patients—especially those on mechanical ventilation (MV) with limited tolerance to hypoxemia—the risk profile changes considerably. Maintaining lung-protective strategies and managing hemodynamic instability are key procedural concerns [ 1 , 2 ].
