Introduction: The clinical interplay between Clonal Hematopoiesis (CH) and B cell lymphoma, especially in follicular lymphoma (FL), has not been elucidated yet. Therefore, we aimed at (i) evaluating the prevalence of CH in a prospective cohort of FL; (ii) analyzing the potential prognostic impact of CH in terms of progression free survival (PFS) and overall survival (OS); (iii) investigating the role of CH in terms of therapy related toxicities in FL patients. Methods:The study analysed 212 FL patients enrolled in the prospective, phase III FOLL12 trial by Fondazione Italiana Linfomi. At diagnosis, peripheral blood leukocytes were isolated and used as a source of genomic DNA (gDNA) for CH analysis.gDNA was analyzed by NGS using the CAPP-Seq technology. The gene panel included 28 genes frequently involved in CH.Data were analyzed using a dedicated bioinformatic pipeline and a VAF of 1% was set for variant calling. Results: At least one CH mutation was identified in 75 cases (35.4%) with DNMT3A (17.5%),TET2 (10.8%),TP53 (2.8%) and CUX1 (2.8%) being the most common affected genes.
