1246-P: Optimal MUAC Cutoffs for Assessing Nutritional Status and Predicting GDM in Pregnant Indian Women

Introduction and Objective: Mid-upper arm circumference (MUAC) is an alternative to BMI to assess nutritional status in resource-limited settings. MUAC cutoffs for malnutrition in pregnancy have been studied, but results vary by ethnicity. Additionally, standardized cutoffs for overnutrition and metabolic risk, especially in South Asians, are undefined. Thus, we aimed to determine MUAC cutoffs to identify nutritional status and predict GDM in pregnant Indian women. Methods: We analyzed data from 2 observational studies at Sassoon Government Hospital in Pune, India (2017-2024). We measured BMI and MUAC at 20-24 weeks gestation (n=570) and diagnosed GDM by fasting 75g OGTT after 24 weeks gestation (n=559) using ADA cutoffs. We used Weighted Kappa to examine the agreement between BMI and MUAC categorization using published MUAC cutoffs. With ROC analysis, we determined optimal MUAC cutoffs for our cohort for each BMI cutoff and for predicting GDM. Results: BMI and MUAC were strongly correlated (R2=0.82), but there was lower agreement between BMI and MUAC categories using published MUAC cutoffs (Fig 1). This improved to kw=0.70 when we redetermined MUAC cutoffs. Threshold BMI of 22.7 kg/m2 and MUAC of 25.9 cm performed similarly in predicting GDM. Conclusion: MUAC correlates with BMI in pregnancy and predicts GDM with similar accuracy.
